There are two different CTRC applications. You only need to complete one.

• Form 1 is purely an application to the Winship Cancer Institute CTRC.
• Form 2 has embedded in it the forms required for the Emory School of Medicine Clinical Trials Office (SOM CTO) for National Decision Coverage Analysis, budget and contract negotiation.
