To make an appointment, call 1-888-WINSHIP (1-888-946-7447) or 404-778-1900 to speak with a Call Center coordinator, who will ask you questions about you and your diagnosis.  They will answer any questions you may have regarding your visit. 

Call Center Coordinators will provide your information to the appropriate Oncology Resource Coordinator, who will call you back within a few hours.  You will be asked to assist us in obtaining records from your referring or current physician. 

The following information will need to be faxed to Winship prior to your initial visit:  

•  Pathology reports
• Lab reports
• XRAY and scan reports
• History and Physical
• Office notes
• Treatment records
• Operative reports
• Hospital discharge summaries
• Copy of your insurance card, front and back 
• Fax to 404-778-1930  

On the DAY of your visit you will need to bring with you:

•  List of current medications and dosages
• Pathology slides
• Scan and XRAY films
• Copy of your insurance card
• Something to pass time such as a book or needlework
